Hello, seems to me, as Your old modem card, AGP, and you need a modem. PCI in a new motherboard. I can not link the "pass" for your adapter. But it seems PCIe cards. PCI will not work. Why not simply buy a modem. New PCI approximately $ 20 -. (Link below) A new card is full, without the adapter and can last longer. Hope this helps Al.
